Application Requirements for Jenan Chetty Cataract Foundation Assistance
To apply for assistance from the Jenan Chetty Cataract Foundation, please provide the following documents:
- A certified copy of your ID.
- A copy of your SASSA card (if applicable).
- If you are employed, a copy of your payslip.
- A sworn affidavit stating that you cannot afford the surgery and explaining why.
- A copy of a report from a doctor on the treatment needed.
